Terms and Conditions

 In line with our company's policies and to ensure the mutual benefit and security of both parties, we kindly request the following documentation. Please note that these documents will be required upon successful approval as a vendor:

1. W-9 Form: In order to accurately report payments made to your company, we require a completed W-9 form. The W-9 form ensures that we have the necessary tax information to meet our tax reporting obligations. 2. Certificate of Insurance (COI): A Certificate of Insurance is necessary to verify the existence of your insurance coverage. It is crucial for us to understand the details of your insurance policy, including the policyholder's name, effective date, type of coverage, policy limits, and other important terms and conditions. This information is essential for us to mitigate potential risks and ensure the smooth progress of our projects.
